My father has different names in different documents. I have father name as Kumar B S in all my documents. But my father's name is Raj Kumar B S, B S R Kumar & Kumar B S in his multiple documents such as Voter ID, Aadhar, PAN, Drivers Licence etc. We are facing some problems because of this dispute in the name. Health insurance benefits, few govt benefits are not being possible. I need a solution for this issue. Kindly suggest what can be done. Will an affidavit confirming the above names are of one person will do.

Your father has to get a declaration from the civil court to the effect that he has been known by these names. And in future he will be known by a specific name. yes .affidavit all names belongs one parson register notery official seal and signature Otherwise file an suit regarding name curretion same as maintained in affidavit ..
