I had my father nominate me in LIC policy but my name was reduced to Deepak S H when it should be Deepak S Hiremath. Now in my affidavit, where I submit that my actual name is Deepak S Hiremath. It is written as "I will be using and signing my name as Deepak S Hiremath". I have always signed as DeepakSH. Should I now sign the affidavit and all other documents as DeepakSHiremath or should I still sign as DeepakSH.
